After at least 15 monster shrines, I can confirm that they are not producing a monster. Just thought I’d mention it.
I am really enjoying the game, even with the bugs. I’m just staying offline for now.

I’m on PC but it worked for me. It does not spawn a monster - it makes a nearest monster to become champion or unique.
If the region around the shrine is already cleared out it won’t do anything.
I don’t remember it working that way but it’s been a few years. Maybe Diablo III screwed me up there. Thanks for the info, I’ll pay attention next time I find one.
