Missing Quest Reward

In act 2 ,Radament’s Lair (Quest) hell difficulty (online).

I went down through the sewers to kill Radament for the quest book to get the +1 skill reward.
I successfully killed Radament, didn’t pick up the book and quickly teleported to town as I was about to die. I clicked fara to heal and my game crashed.
I reloaded the game hoping I could return and kill Radament to reclaim the book with no success on dropping it. I completed the quest assuming Atma would give the book if something had occurred. No success in that either.

Is there anyway to resolve this occurance?

Edited: 12:11am EST Mine appears to have been resolved - I hope all others experiencing this situation can get it resolved.

I had the same thing happen. I killed Radament to complete the quest in nightmare. I saw the book drop but my inventory was full and then the game immediately crashed. I logged back in and started a new game but the quest was completed and I had no book to get the skill reward. I went and killed Radament again and he did not drop the book.

How was your issue resolved?

I was able to resolve this issue by going into lobby and creating a new game. Upon entering the new game the quest was active again on my quest log.
