Think you should read Mal in its entirety before you disappoint yourself.
Casters do not have any way to apply PMH. You can cheese and have a weapon switch with like a bow to shoot whatever and apply PMH every couple minutes or use merc with it.
this also wont work, merc’s cant apply it either.
the cheapest way to apply it as a caster ( if you Really wanted to for some reason)
would be a short war bow with 4x eth and a mal
( the 4x eth will allow you to not need any attack rating and unlike “ignores monsters defense” works on bosses)
you could do it with 5 soc melee wep as well with the same runes but casters generally prefer staying at range, hence the bow
widowmaker with a mal also works, but the 155 dex req isn’t something your going to generally reach with your caster
“short war bow” is the bow is the least stat req that can reach the 5 soc needed
but it also won’t work on any of the key bosses ( the tristram bosses, lilith/ uber duril, uber izzy)
(it does work on dclone)
since im discussing this… i should not iron golems can also not apply it.
also like guitars said.
mal only apply prevent monster heal when put into a weapon…
any other piece of equip it will give magic damage reduced by 7

i understand that… i did fight 3 times witch dclone and could not kill him because of his heal… if i will put it in lidless eye and try to hit him with it - PHM should apply?
bow + eth is good advice.
last time i did get a sword with phm and hit him and after switch back to staff he started to heal again… if i dont swich effect should stay?
no, mal does not apply prevent monsters heal in shields, armor or helms, just in weapons

i see the confusion… yes if u put it in tals wep… and swing it at the enemy and SOMEHOW land a hit on them… it will apply.
but you wont be able to hit anything with it becuase your chance to hit will be 5%, with no attack rating
and it cant be applied by elementa skills, just your melee swings.
the confusion here is even though u said “lidless eye” which the eye meaning tals wep
to every veteran d2 player “lidless” means “lidless wall” the +1 skills grim shield that the best shield for casters short of spirit if u want the str for monarch.
so no one read and understood the “eye” part.
just that “lidless” and everything thinks shield

I generally use Edge bow for it (because I always have one anyway), although been a while since I played sorc, so as SwiftKitten said you may have issues with hitting him. Angelic combo though should put you somwhere in 1/5 or 1/4 arrows hitting.

I’ve never used this method b/c I mostly run melee, but the nice thing about Edge is that it also has poison damage, so you have an easy visual cue that you’ve landed a hit and applied the PMH.
I think you meant to say “but it also won’t work on any of the key bosses”, may want to edit to avoid confusion!

The easiest and cheapest way to apply Prevent Monster Heal on Dclone with a sorc is to use an Edge bow on switch. Get a 3 socket short bow (damage does not matter), which can be vendored from Charsi in normal difficulty, then create the Edge runeword (Tir + Tal + Amn)
Edge stats:
Level 15 Thorns Aura When Equipped
+35% Increased Attack Speed
+320-380% Damage To Demons
+280% Damage To Undead
+75 Poison Damage Over 5 Seconds (Tal)
7% Life Stolen Per Hit (Amn)
Prevent Monster Heal
+5-10 To All Attributes
+2 To Mana After Each Kill (Tir)
Reduces All Vendor Prices 15%
Shoot Dclone with Edge bow until you see a red lifesteal swirly on your character (It will most likely take several attempts due to crappy attack rating), then switch back to your main skills. Just don’t let Dclone get too far away from you or TP to town, or you’ll have to reapply Prevent Monster Heal again.

sure thats cheaper than the mal +4 eth bow, but certainly not as reliable because like u said, u need to worry about attack rating
Too bad PMH doesn’t work with mercs.

Dont worry about ar…it always works after 3-5 arrows and this time you will stand him easy 
(And later you go to gheed with your edge bow - perfect)

PMH does not work on mercs. LOL
Better to use a weapon, but caster hitting with a weapon is a joke, unless you are like some hc lvl 80 sorc, who just saw Dclone when doing the yet another Hell Mehpisto run on day 1.
This helped immensely. Just got my very first anni solo with a l84 holy shock zeal pally wearing not bad gear but none of the specific items I’ve read about for hunting ubers. Had a 5os Ettin axe laying about so plugged 4x eth with a mal and charged in with my smoke mage plate. Took a while and about 40 rejuv potions cut him down in the end. Build was Irathas set, Gore Riders, Ancients pledge and an Atlantean. Though not sure how useful my merc was in the end. Insight has been carving up as I’ve just been working my way to finish Hell but not sure if he was even landing hits. Thought it wasn’t possible with my gear/ level but your suggestion worked wonders. It is possible

normally as a melee character vs ubers you use open wounds and crushing blow (your gores have both)
for the duration of the bleeding their hp regen will be set to a negative value, it doesnt last as long as as prevent monter heal, but it does work on every monster in the game, including trist ubers
another thing you can use for longer fights vs enemies with high defence ( that no one does) us to use things like soul drainers with things that say
-50 To Monster Defense Per Hit
its generally perfered to just increase your own attack rating, as this wont help for short fights as you still need to hit the enemy a few times to stack it up and during that time your going to miss a lot, but for bosses like ubers that normally last a while, after say 10-20 hits your going to have the 95% chance to hit
its an extremely rare mod only found on 5 items
the only non-weapon of which is soul drainers
but the cheapest way to get it is malice runeword
it wont deal much damage, but you can trigger useful mods pretty quickly as soon a a few hits land before swapping back to your orginal wep.
like i said earlier though you do need to hit the enemy a few times with the wep before the defense starts to lower
