Completed in both normal and nightmare just now, quest shows as completed but did not receive the skill point rewards.
Same thing here. Just killed him (but forgot to talk to him before portaling back to town).
Quest is shown as completed, but I did not receive the extra Skill points and Tyrael does not offer them anymore.

I leveled up before talking to tyrael. Maybe that caused the bug?
