I can farm the countess for rune, and i have the exact same rune if i go normal, nightmare or hell.
Is that suppose to be like that or is it a bug?

You can get up to Ral rune in normal, up to io (Not to be confused with Lo) from Nightmare countess and up to Ist from Hell Countess.
Countess is the only monster that rolls rune treasure class directly unlike other monsters that roll Good Treasure class and rune treasure class is a sub TC of the Good TC.
So Hell Countess’s normal Good TC can drop up to Lo rune if it rolls the sub Rune TC but it’s very rare.
Although HellCountess’s special rune TC can roll up to Ist, Ist is very rare still, it’s like 1:300 chance (Not exact but you can look it up). In general, you will have to farm Hell Countess many times to see Lem or higher
EDIT: Also, if you want to maximize countess’s special rune TC, you want to farm her in player setting 1. That’s normally when you can get up to 3 rune drops. If you raise the player count in game, her special rune TC are replaced by item TC, so you will get more other gear item and only 1 special rune drop.

not sure 100% what you mean exactly
the higher level mobs you fight the better the rune Drop is, so nightmare will drop on adverage better runes then normal diff, hell will drop better then nightmare on adverage

Hell countess just drops eth and sometimes 0 runes
hell i had my Hell Hellforge drop 0 runes and gems for me (Bug as its 100% sopos to drop gems/runes but blizz dosnt care nor the whiteknights)
nightmare countess? Thuls and above a few puls once in awhile SO much better then hell countess by a long shot but never above puls.
while ONCE once in a great while hell countess will drop keys for me
the games loot system is terrible tho.

What i mean is, i just try the countess, 5 time in normal/nightmare/hell.
That was always the same kind of rune who dropped. eth tal tir ral shael ith.
Nothing else, i had the same ral in hell that i loot in normal.
The high runes just have a significantly worse chance to drop, that’s all. So you will see most of the times the same drops in hell as in normal/NM. Countess can potentially drop up to IST in her rune drop table but there is no guarantee to have an IST after 100 runs. I own around 30 keys from her (that makes around 350+ runs or so) and never saw an IST.

Based on the number of terror keys I have I killed Hell Countess 2-300 times. The highest drop I got from her was a Lum (only once - a few days ago). She dropped lower mid-runes (Sol, Dol, Hel, Io) at least 2-3 times each.

I am also getting really bad hell Countess drops, typically Tal, Ort, Eth. My most recent run got me a single Eld. This is probably just bad luck not that runes are dropping anywhere else for me. After a month zero runes other than a Tir from LK.
I’m not doing it again.
It’s worth nothing and she’s immune to cold and fire, my 2 main…
I will find another path for farming runes then.
Farm countess for keys and low runes, she is absolutely horrible for farming high runes. To get high runes you need to farm hell, and monster density is highest at cows.
