Is character rename possible in D2R?

For offline, you can rename the saved game files. It will rename the character in game.

How was it done in original D2?
Does that still work with Modern Bnet though. The game is not on the same databases and infrastructure anymore.
For me the rename of the files work but not 100%. It changes the name in the selection screen but the old name still shows on the stat screen (i think it was). Still it is a good enough and easy way to change it.
Correct. Renaming the files will change the character name at title screen etc. But the original name will remain when your character talks and it shows the text in the bottom left and on stat screen. I’ve looked into this myself and the only answer I ever found was using a character editor which only works for original D2 saves not D2R. So unless it’s an offline save you are pulling from D2 you are out of luck I’m afraid. The closest you will get is just renaming the character and dealing with the text window name/character stats screen not displaying correctly.
