Gray maiden spear + Insight runes = no runeword.
Haven’t tried with other polearms, but it doesn’t work with maiden spear.
While the game’s item base types are often cryptic, this is not one of those cases, and it’s on you for not checking. A maiden spear is a Spear class weapon, not a Polearm class weapon. The runeword similarly won’t work in other Spear class weapons like a Pike, Lance, Brandistock, etc. To keep them separate in your head, think of them this way - Spear class = thrusting weapon, Polearm class = slashing weapon.
