This is on PC, offline softcore. I’ve seen many people reporting this on other websites, but I wanted to let the team know as well. I was up to about 35% complete for the Chronicle, and then today I noticed it was reset back to 0%. It only shows items I found during terrorized Travincal (4 uniques) earlier today. I’m assuming it had already been reset recently and then those are the newest items post-reset.

I can confirm that all chronicle progress prior monday 16th is lost although i still have some prior found items in stash.
I can also confirm that there is more than 1 chronicle: at least 1 online and 1 offline. My offline chronicle is blank since i played RotW online only while my online chronicle still has entries.
All uniques and sets found so far came from bosses. The last action before i noticed the progress loss was building a Stealth armor for a new Warlock.
Do you remember if you found any uniques/set items from a weapon or armor rack or a chest by chance? I’ve had mine fully reset as well and I think it was due to identifying an item from a weapon rack. I believe this is the running theory for the cause of the issue as the Chronicle registers what monster the item dropped from but may not be properly coded for items that drop from racks/chests…hope this gets fixed soon.
This must have been what happened to me. I remember getting unique dagger (gull) from weapon rack. Lost about 50% unique 70% set chronicle progress :D.. cursed dagger^^

It happened to me as well. And I did find a Set item from a weapon rack in Act 3 yesterday.

Mine just reset. I just refunded.
For offline players: do not identify items from weapon/armor racks.
Still NOT FIxed in latest patch.

I had the same problem. I farm lower kurast on offline and after identify set/uniq i lost whole chronicle list. It’s so frustrated becasue i already started to make runewords and i lost those achivments. Im not playing till blizzard confirm this bug is fixed. Yesterday was new patch but people giving feedback that it’s still not workig. Im so sad beacuse i wanted to play this weekend. 
Damn… Is it legit info? Your chronicle reset again after new patch? 
Still happening as of today. SSF offline chronicle wiped. Wish they would warn players of this with a pop-up or something when they obviously knew it was going on.
Just happend to me too on PC offline. Just great.
My unique and set item progress has been maintained but I created a Last Wish rune word recently that I did not get credit for. I made it for an ubers barb and my chronicle still said 0%. So I tested it out and made a Rhyme shield and now it’s sitting at 1%..
