I was in NM Cows, had just killed first pack and was fighting in the next pack and disconnected. After a while I got back in the game, a new game, and all my gear was OK, but the Horadric Cube was missing from my inventory…

This happened to me as well, while I was in cows, I also got d/c and dropped from the game, upon rejoining my next game I had no cube.
Interesting bug, related to cows, they are completely unaware of, that did not exist in ogd2.
Happened to me twice now. Just yesterday I lost a Token of Absolution due to unfortunate timing, game didn’t register it.
This has happened multiple times to 1 offline character and it was not cow-involved.
Exited offline game, upon starting offline game again, the cube is gone. I mean at that time the only loss was the annoyance of having to go retrieve it from the act 2 quest again, since nothing was stored in it.
Happened to me to. I just want back to normal and get it again in less than 90seconds.
Getting disconnected, crashing…etc doesn’t erase a game that’s already been in existence for over 3 minutes. This Any items in a cube drop back onto the ground should a cube disappear or be dropped on the ground.
My point is, just go back into the game and find your items then go get the cube on normal.
Happened to me twice, now I make sure to not leave anything Valuable in it (Just in case). Got dc’d and when I come back in game - no cube. They really need to fix this game.
Kill the council in trav and youll get another.
The problem is, people sometimes keep off weapon swaps or armor swaps in cube, for situational problems, and when this glitch happens, and the cube poofs, any items inside it also poof.
I believe it is the Global DB vs the local DB update.
I was in a private baal run and was given an TGods belt.
Next game I joined, the TGods belt was gone and my old rare belt was back.
I went and looked in the stash and it was not there, nor in cube.
I then noticed my skills and stats had not been updated, tho in the previous run when I got the TGods belt I had leveled and assigned the stats/skill pt.
The Data Base update caused my lost TGods and unassigned stats/skill.

Just like your Cube being gone from Cow game is my guess.
