The Maw exists for beings that are so reprehensible that places like Revendreth cannot redeem them.
So a small number of people are sent directly there, others go to Revendreth, refuse to atone and then go to the Maw, there may be other places that give new souls a chance but send the failures to the Maw but Blizzard only showed us four of the afterlives.
