I didn’t get removed, but I joined to heal and on first pull, the tank overpulled. His HP dropped to 20% in one hit from the pack, and I said “Nope” in chat and left.
That count? 
Got kicked from wailing caverns, after we pulled the last boss, just before he died, so i wouldn’t be able to finish the quest/get the completion.
The reason? I suggested the whole group go through the dungeon together, where instead the tank went one way and the dps went the other.
It was a good induction into what retail had become.
Yeah, sounds like the OP was spending their time inspecting / backseat tanking instead of doing what they were actually in the group to do.
Got kicked from a m+ WS because I didn’t port into WS staging area since it does it automatically when you put the key in. 
I’ve been booted twice.
Once was because the healer hated healing melee in a group that was 4 guildies and I was a random pug from the lfg tool.
The second time was because they didn’t think I was doing enough healing… on a rogue. 
(I wish I was making this up.)
I got kicked from a tw on my mage once. I was terrible at mage. I deserved it. End of story.

Booted for two reasons on alts, put myself on follow after letting the group know I was going to use the bathroom/pee really fast [tiny apartment so would have been quick] apparently that’s illegal and you have to invest in a catheter so ate a kick and the second was just for being a hunter [this was back in CATA had growl off and cower on so pet wouldn’t steal threat from the tank and didn’t mass pull] apparently the healer got tilted and super catty seeing another night elf lady, and after that day just stuck with PVP on that alt. ._.

Not been kicked much, but one was really funny! I was MM no pet, and was told “hunter stop pulling with that frost shot” before I could respond I was booted. The person that was pulling was a frost mage 
always blame the hunter
I got removed for being a hunter once lol
I joined an in progress End time dungeon back in Cataclysm as soon as I entered I am going to assume a hunter they kicked before me wiped them because someone said

I get kicked rather frequently because I’m a disabled player.
I only have one useable hand and so I have to move, target, and cast all with one hand. And, I can’t type to explain the lag while trying to move. Thus, I tend to lag behind the pack. Despite using the arrow keys to move, and my spells being on my num pad, I can’t use tab the way most people do for mobs without having to completely cross the keyboard. So I tend to target with my mouse instead.

So for the longest time I was running the obsidian skin axe, and the AoE that comes out of that has a huge radius and accidentally pulled some mobs and wiped the group. I didn’t get kicked per se, the group just sorta disintegrated.
Or when the tank pulls the whole street on the first pull and dies within seconds. Gets grumpy and blames heals.
This happened to me, I /ignored my group teleported out of instance and just did quest until they were able to kick me.
The amount of abuse thrown at healers is crazy.

My friend reported me to be funny, with an obviously false reason, and to their dismay, it actually went through.
… Though to be fair, I’d done exactly the same thing to them months prior.
I was kicked out of Blackfathom Deeps because I kept on failing the “jumping from pillar to pillar game”.
I fell into the water over & over again and couldn’t get back up fast enough.
I’ve had people recognize me from the forums and boot me
I thought it was funny
