Why aren't mistakes punished like they used to be?

Heroics and M0s need more focus on personal responsibility. In fact 2s through 11s could use the same treatment. We need far more “you failed the mechanic so you are punished” instead of the current “just make it harder for everyone else” system we have now.

Adding in more one shot mechanics would punish the whole group. Especially once you get into M+.
