Is it going to be simply the world map as it was in vanilla? Will it be the vanilla map plus new zones specific to C+? Will it include any zones from later expansions?
How will that affect battlegrounds and arenas (if arenas are included)?

I remember Outland used to be a level 60 zone but instead of a patch it became an expension. This was never official but old data mining from back in the days.
