It’d be a third scourge invasion. The first one happened in vanilla with the release of Naxxramas, the second one happened as part of the WotLK pre-expansion event.

My understanding is that the helm is what makes Bolvar the Lich King. And as Ner’zhul’s presence was in the helm, I would think a little bit of Arthas’s presence remained as well. It doesn’t make Bolvar Arthas just as it didn’t make Arthas Ner’zhul, and since it wasn’t intentional, any outside presence in the helm isn’t enough to completely overpower Bolvar.

With that said, there would be some merging going on, enough to make Bolvar seem like a different person, along with the strain of controlling the undead. I wouldn’t be surprised if the only thing holding Bolvar together is his idea of one last service to Azeroth.

The Helm of Domination controls the Scourge through its own enchantments. Ner’zhul had a presence in the Helm because his soul had been bound to the armor and Frostmourne, and Arthas destroyed that soul. If any presence still exists in the Helm, it would be a retcon or new information.
