There have been instances of and implied mak’goras where wanting to be a warchief is something that is not required to take part, making it seem like once a mak’gora is done, you may deny the title of warchief.
You are right I suppose that it is optional, and as a result of not doing so, Saurfang is or should be looked at as equal or less than Sylvanas.
