Inside of a city you can also have military targets. For example a fort INSIDE of the city.
You hit the fortress with all you can and minimize the loss of life in the city.
This is not a hard concept to grasp.
This is why in war when you want to capture a city there are strategic military targets to capture rather than indiscriminately destroy a city because “lol city is a target”
