In a time when disability is identity and some people resist cures (let alone treatment) on the grounds that treatment is ableism because it implies that what they have is something that doesn’t call for treatment, some people would rather see a phobia as a sign of their uniqueness and would want the world to accommodate them rather than get help to overcome their phobia.
I have a crippling fear of heights or I used to at least and I’ve not let it stop me from bungee jumping or trekking in the mountains. But at one time I couldn’t climb ladders without panicking. The steps you outline to overcome a phobia work and are proven. It’s pretty unfortunate that they will not be recognised as helpful and compassionate.
