I dropped the quest item around lvl 12 and abandoned the quest. Now that I am lvl 14 I went back to the quest giver in Ironforge (Muiredon Battleforge), but he no longer gives the quest item (Symbol of Life), so I can’t continue with the quest chain. Closing/Reopening client doesn’t resolve issue. Playing on (NA) Bloodsail Buccaneers, if that’s useful info.

I’m currently in the same position. Were you ever able to receive the quest again or am I doomed?
To clarify, you are having this issue in Era, correct? Furthermore, have you checked your trainer to see if they offer the spell?
