“Life is as empty without chaos as it is without love”, people were perfectly aware that some would be inconvenienced but it’s something that would have happened for the 3rd time in a 15 year old game for only a couple weeks, so I think those very selfish people can manage.

So how did it work? Did some NPC ghouls pop up and infect players first? What was the chain reaction how did it play out in PTR?
There’s a reason compromise is a thing big guy.

Well, if you are sadistic as I am. You’ll probably understand. 
There were crates of plague grain that would appear randomly through the city that players could interact with to become a ghoul and it would spread like that.
Players also had a toy that they could get that when used destroyed crates of plagued grain to prevent ghouls.
