We entered Visions of Orgrimmar and attempted to open a chest. This freezes the scenario but we can still move around. We cant interact or attack anything. After 5 minutes we get disconnected. Then cant log in the game for 10 minutes it says name already exists. Once we get back in game we are out of the vision back in the heart chamber. The vessel is used and need another to enter again. This has happened 2 times in a row. 4 vessels wasted.

Can confirm, this happened to me twice because I didn’t realize attempting to loot the chest was the cause.
