Since Vanilla, there was a zone called Crypt under Kharazhan that could be accessed through various tricks. There were many areas to explore, such as the infamous room The Upside-Down Sinners, where human corspes can be foung from chains. During Legion, the Crypt was renamed the Forgotten Crypt, and made accessible when you solved the Riddle of the Lucid Nightmare. In my view, this makes the area canon.
So, what is the lore behind there? Where did those humans come from? Did Medivh have them executed?

Kara wasn’t originally going to be instanced and once had a bigger model. The crypt was originally part of the original Kara and would have been part of the raid.

The Crypt’s entrance was used by Azeroth’s first Necrolyte who was an Eredar Sorceress who plotted to usurp Sargeras and was killed by the Guardian of Tirisfal.
Aegwynn using Aluneth built Karazhan under Sargeras’s influence even if he couldn’t compel her to retrieve his Scepter.
Karazhan is likely just an insult to the Eredar with the Deadwind Harvester who plotted against Sargeras! She may have created a vast Underground Necropolis yet no one would notice it with Karazhan standing tall right in front of it!
Logistically the Council of the Black Harvest’s Leader(the Player Warlock) would be the inheritor of the Crypt and thus it’s master owning the largest fortress on Azeroth aside from Ulduar itself. In the Real World the Warlock would install an entrance into Duskwood larger than Karazhan itself!

Location of spooky necromantic magic used by the First Necrolyte (Eredar) using a magical Fel-staff with the soul of a Dreadlord that led to the a trans-dimensional nexus that led to Karazahn being a “weak point” between planes of existence.
Karazahn’s nexus is a necromantic nexus.
Ergo given that the Maw has cages “hanging” upwards like the Kara Crypts, there was some speculation that somehow the Maw connects with Karazahn’s basement/crypts the same way that Karazahn’s attic connects with the Twisting Nether and Library connects with the Great Dark Beyond/Void.
