Lifebloom: Mouseover or Self on Modifier?

Trying to make a macro that will cost Lifebloom on mouseover (or if possible, the person in the tank role in a 5 man, but not sure if that’s possible?) or if I hold alt and hit the key it will cast it on me instead.

Target Enemy Targeting Friend: Cast on Enemy’s Target
