Well, it’s stated that his appearance in Return to Karazhan is his actual self, and we see him fly towards the Nether.

So the guy is probably roaming around, I don’t think he is in the Shadowlands.

He’s in Karazhan. We see him at the end of the Legion Karazhan dungeon, and Wrathion talks to him in his journal.

Whatever Medivh is, he ain’t dead.

I suspect that all appearances of him after his death are that of a ghost.
