A real life example, a major automotive maker designed a very nice vehicle, then they found that a freaky spider in certain areas of the world, that seems to be attracted to fuel vapors, liked to crawl into a system vent to use as a nest and plug the vent with webbing causing functional issues. The vehicle was recalled twice to try to implement fixes to prevent the spider issue.

This doesn’t mean that the vehicle was designed poorly by the engineers or that the vehicle was bad.

Expecting the engineers to foresee the behavior of an insect is unrealistic.

Just as your expectation of a perfectly bug free game is unrealistic.
