I am having this issue as of last week and this week. Always around 7pm when raid time starts. It is so bad that I have had to bench myself. I have tried different computers, different connection (1gig Fiber and Verizon 5g Hotspot) and it keeps happening.
Same problem here. My world ms remain between 90 ms - 110 ms, but my world ms spike to up to 10000 out of nowhere. I’ve tried everything and nothing seems to work, and this problem only happens with WoW. The only thing that worked for me, for some reason, is changing realm when this happens. Once I change realms, for example, from Moon Guard to Wrymrest Accord, and come back, the issues gets solved for like 5 - 10 minutes. An hour if I’m lucky, then it comes back to 1000+ ms.

Having terrible home and world or home OR world ping the past few days and it's unrelated to my ISP. Exiting WoW and relaunching always fixes it, but temporarily. It reverts back soon after. My ping is usually around 6 home, 6 world.
You’re having spikes of 159 ms to the router, and 388 ms to the isp and continuing. 1% packet loss from the 1st isp hop onward.
You may think it’s not your isp, but it starts on your home network.

Going to agree with Zungar on where the problem starts. Can ask the service provider for assistance doing a factory reset on the modem/router, or try testing the connection with a different device. Though if neither of those options work they may need to send a truck out for a more detailed test.
If they try doing a ping test while on the phone, let them know that is not a detailed enough test and ask them to send someone out. A ping will just do a single packet of data whereas the person in the truck has a device dedicated to testing end-to-end connections for faults. It’s similar to the winMTR test but way better. Can’t offer that through the forums though since it requires a physical device to be attached to the network cables.
