I can’t point you to what quest, but this sounds to me like you are stuck on some kind of quest that applies that appearance based on the fact you only have the wrong appearance while playing and not on the character screen. There are some quests that have cosmetic affects on characters that are retained even if you drop the quest, but I would check your quest log just in case to see if you still have anything that sounds like it might do something like this.
