Which one do you think is?
Often when I hear people commenting on this subject. “The Insane” or the 25 raid ones are often mentioned.
However in my opinion " Accomplished Angler" should be considered one of the hardest, if not THE hardest, simply by the fact that only a finite amount of people can get it done: A maximum of 106 people per year per server.
The insane isn’t hard, just tedious.
I’d say to actively try and get, “we had it all along cough” or any other rare case pvp achieve.
Seconding “We had it all along cough”. There’s no way anyone has gotten that achievement on purpose, especially pre-nerf

Throughout all of TBC, and still in the game, the STV fishing contest can be done about every 10-15 minutes, with some weeks taking a while longer to reset the first time.
However, it does include the wishing well coins, which are a massive PITA.
I would say the Ulduar Hard Mode meta achievement or the PvP Battlemaster achievement are the hardest to get.

It really depends what you mean by hardest. Do you mean the achievement that the least percent of people complete? Or actually challenging to do
I got this on my main account pre nerf and it literally could never happen in todays pvp environment because it requires constant trading of that third base right to the end.
In todays ABs ppl quit the second they are down 3-2.

I’d say the Undying is pretty hard just because it takes everyone to pay attention and well…people are idiots.
