According to this,
the first kill on the Undermine World Boss should give a piece of loot along with the crests, rep, any money etc.
but it did not for me and I have only killed it once since the patch went live. Is there a change I can’t find anywhere or did this bug for me?
Seems if you did it last week the lockout did not reset; go on a toon that did not last week. Unfortunately you are probably account-saved for the week now…

Interestingly I don’t think it I did it on that char last week. Because I got everything but the loot piece. My chars that did it last week don’t have the Undermine World Boss available to even kill as a quest.
So, I am guessing what you said is correct that it just did not reset from last week.
