If I ever make a healer I might change my UI though. I would want the party frame or raid frame in a different t place much more convenient to see.

I like the aesthetic of it and think it nicely suits the fantasy of the game. A lot of people’s UIs, probably because they use Elvui, just end up looking like they came out of a sci-fi game rather than a fantasy one, at least to me.
The organization etc. can really be lacking though. My UI still looks pretty vanilla at a glance, since I just made some adjustments to give me more information/more easily accessible information.
“The UI is too small” is a complaint I’ve never heard before. Sounds like your friend either has never played an MMORPG using an actionbar before OR they’re using a 2k-4k monitor, which does indeed end up with things being smaller (which is a boon for me, but if they have really bad vision, maybe not? Tell them to turn up the UI Scale if necessary).
Also, refusing to use addons that would immediately fix their issue is stupid. It’s literally drag and drop into the addons folder. Done. That’s it. It’s not like some Skyrim modding where things have a page of prerequisites from 3 different sources which each potentially have their own prerequisites, all with varying installation methods, including an entirely separate launcher and mod loader.
All of that said, WoW’s base UI bugs me too. It doesn’t make sense (have to look in too many corners of the screen for relevant information), and on top of that, it looks like it was designed by the guy who made Diablo 2’s UI or something. As in, incredibly dated and refusing to modernize. “Fantasy” doesn’t have to mean “Overly textured, bulky, and entirely grey-brown”.
It WORKS, but it’s not GOOD.
The base ui is absolutely rubbish but I like being able to customise it however I want and the extra “player skill/knowledge” that allows.
It means a better player with a better ui will perform better
