1 chance per week, not per difficulty just 1 chance. If you can do it on a higher difficulty, do that cuz its slightly more chance, but you can only get the chance from the first kill of the boss each week

It does not say the drop is once per week.

We have had TONS of confirmation from people who have done H THEN N or N then H and gotten the drop in the same week!

The item will be very rare on Raid Finder difficulty (when available) and slightly increases in drop rate with each difficulty.
