So, is the “fill the bar” quest a daily, or a one off? If it’s a daily, can it be filled by killing a Rare (since they drop 30 black stones)?

This isn’t a bug. You got the stones before you picked up the quest. The repeatable doesn’t start until after you do the quest and stones you picked up before getting the quest don’t count.
Unfortunately, you went on a Rare hunting spree before finishing the questline. Just finish the questline and you can do the repeatable.
You lose nothing except having to spend more time gathering stones before spending them. It’s “bugs” like this that are the reason we are told to go to WoWhead first for every ticket.

For the first turn-in quest, only the ones you collect after you pick the quest up count. However, once you’ve turned that first one in and have gotten to the repeatable ones, then you can use those to clear out your bags for commendations.

Yes, you do. And it’s actually a daily thing. I had to do it yesterday AND today before I could change the stones for currency.
Yeah, on my lunch break, I ran through the initial quests on a charcater I have not done it yet with. The turn ins did not appear until after finishing the bar quest and the bar quest did not appear until doing a few other quests. And I was wrong on the bar quest NPC, but not the NPC that accepts the exchange. It is the Quartermaster.

This^ … Also, I wouldn’t drop the quest; untracking it then retracking it sent me from 20% to 60% immediately .

This one seems bugged a little. Yesterday I had no option to turn in either one at all, then when I went back today I could complete them. I didn’t drop and retake or get more stones, the option to turn in just appeared. Then after you complete the initial quest, the dailies to turn in 25 stones as often as you get them appears.
you have to untrack and track again, then the bar appears
You are locked out of the vendor in ICC if you haven’t done the daily. You can not do the daily if in a raid group. It’s a little counter intuitive, as the rares require large groups to take down.
Untrack and re-track the quest in your log.
