the item from hoa is not a bug. the vault give you loot from all dungeons regardless of what you have or have not completed that week.

According to this post it should not happen. The GM will tell you to file a bug, but they won’t fix it.
If history is any indication, there will be trolls in this thread telling you otherwise. You can safely ignore them.
In fact, if you do see people telling you otherwise, I would recommend clicking on their name and then clicking on “normal” to change it from normal to “ignore”. These people keep coming back to grief and post wrong information over and over again.
It can happen and isn’t a bug. He unlocked 2 vault slots, each matching a ring he already had equipped.
The no duplicates applies to the vault itself. Meaning the three slots will be different pieces from each other. There can be duplicates from previous weeks vaults.
I kind of agree with the idea here. Just bring back extra roll tokens. I can’t think how many times I’ve waited 5 days just to get a slot I just spent some currency to upgrade. I won’t even upgrade things now until I see what’s in the vault for the week.
Nope because I only ran 1 mythic last week and it was Mists and the item in my vault was from another dungeon. And no I didn’t run another and forget because only running 1 isn’t hard to forget.

I think you are replying to the message I posted before I got 2 GM responses. I’ve updated the OP with those responses. Basically, it’s accomplish-once-unlock-forever mechanism when it comes to what goes on the loot table.
