Heroic mode, on my hunter, action bars went totally blank when I was pulled into the twilight realm at the start of the Ultraxion boss fight. I use the base UI, no UI addons. Reload did not help. I ended up autoshotting the boss dead and then my bars came back. Tried dragging spells to my bar and/or using spells from the spellbook and neither would work.

Had this happen two weeks in a row as well.
/cast Fire Blast from spellbook to bypass it.
happened last two runs on my rogue also.
yesterday was the 2nd run.
all bars except for bar 1 (which i only have skyriding spells on) dissappear as i get “pulled into the shadow realm” as the fight starts.
meleed the boss to get around the issue.
bars came immediately back once I killed the boss.
I use bartender addon.
i’ll test with another character / without addon later today.

I use ElvUI and this happened to me as well. Abilities came back immediately when the boss died.
I’ve had this happen before also. Was fighting the headless horseman, years ago, and as soon as I zoned in to the instance, all of my action bars had been completely wiped. Even after zoning back out, they were empty and I had to reapply all the action buttons. So this bug has been around a while. I wouldn’t expect a fix any time soon.
This is also happening to to me with all my characters during the Ultraxion 25 heroic fight.
I don’t use any interface add on’s

Still happening with no addons loaded. Extra action button does not appear and using a macro to /click ExtraActionButton1 doesn’t work.
Action bar 1 is replaced with a blank bar and can’t be swapped out.
On the laughable chance that Blizzard actually sees this, it appears that the Ultraxion encounter is being considered vehicle combat. I get the little “leave vehicle” icon even though clicking it doesn’t restore the bars.
Seeing the same issue here. Has happened 3 times to me now.
I can confirm this still happens, both shaman (70) and mage (70) just now on 25H , I suspect it’ll be the same for all my other characters too.
Not sure if this is what’s happening to you, however there are hotkeys which will switch your main action bars, say switching action bar 1 to different action bar which can be empty. I found I would often trigger this accidentally, so have remove the hotkeys for this since I don’t rotate hotkey bars.
No I have more key bars then that, and I have no swap key. NO other fight dose this.
Thankfully I discovered I can right click on the bastard, and auto shot KO him.
