The quest chain for this goes:
Drop from the final boss of Wetlands Excavation → quest “Lost Relic Carry” → turn-in at Prospector Whelgar → next quest is Prehistoric Prism
But somehow, the quest item “Titan Relic” is disappearing from players’ inventories. And there’s no way to re-acquire it since it only drops when you have not completed the first quest (Lost Relic Carry). Dropping the quest Prehistoric Prism and picking it up again, does not provide you with another Titan Relic, so the quest chain is softlocked there.

I abandoned the quest thinking I could just get it to drop again, but the quest never dropped again. Ran it 3 more times after. I really hope blizzard not only fixes the quest turn in, but also allows people to get it again if they’ve abandoned it.
Just make sure not to abandon the quest just in case because…you know… they’re Blizzard

yep its broken the dwarf does not give it back to you when you turn it into him. he just steals it.

I just submitted a bug report through Issue reporter the same thing has happened to me and I have run the dungeon multiple times
I can confirm i just seen someone with one of the quest rewards. He with through the quest chain and it worked for him.
