Why is this for keyboard hotkeys only? Is there a way to get it to work with mouse binds as well (without addons)?
Macros. Like it always was before whenever retail introduced the mouseovercast.

Even in retail its like this, there’s an addon called Clique that does basically the same thing but lets you use any keys you want. (I heal with a 12 button mouse)
There is a version for Forever out, but I’ve heard that its not quite working as it should yet. Could always try it though and see.

I think the @mouseover thing is because of the client based on the Midnight client? Not sure, or they added it in.

I used to use Clique for a long time. seems whenever Blizzard tries to put an addon into the base UI it always falls short and still needs an addon
I still use mouseover macros because they provide a priority system based upon my preferences.

Do you have a program to bind your mouse keys?
I use a corsair scimitar and I have my 1-12 on my mouse bound to CTRL 1-12 and have no issues with binds.
