Now hold on even small indie companies can be pretty competent look at Wayforward or Yacht Club Games.

Not only deploy capital but also generate cash flow beyond their shop. In the past WoW was able to generate cash flow because they actually cared about subs.
When they announced they don’t look at subs anymore that is when it indicated that something was heading in the wrong direction.
It’s a reference to basically any time a gamebreaking bug is found, and that a company as large as Blizzard shouldn’t be letting them get through with the frequency that they have been as of late.
Perhaps they shouldn’t can half the quality control and assurance department.
A quick google search shows that it didn’t actually start with Blizzard, but rather Riot (who make League of Legends) as early in the game, they used a lack of resources (i.e. Small Indie Company) as an excuse about why they couldn’t fix bugs.
Eventually someone used the same bug in reference to Blizzard (specifically Hearthstone IIRC) and it just kinda spread from there.
