Despite finishing Season 3 in Plat on Tank and Support, the game says that I was gold last season. Due to this I received fewer competitive tokens and did not receive the “Platinum Role Challenger” player title. In addition to this, in the rank decay screen when pulling up the competitive tab for the first time this season, my new ranks were the exact same as the incorrect ranks from S3.

Same, I finish master 5 but get diamond rewards and now it says I finished diamond?

The Devs have acknowledged this bug: Overwatch 2 Known Issues - April 11, 2023. However, we still need to keep spreading the word about this bug so that they actually fix it unlike the bugged career profiles and missing end-of-season rewards from other previous seasons like Season 36.
