There have been a few heroes that got literally broken so badly they were removed until they were fixed.
Mercy 2.0 before all the nerfs.

people will scream brig, but… idk Duel rez mercy was a huge pain in the butt. I REALLY wish I played her just to get “huge rez” achievement when mass rez and duel were a thing. It’s almost impossible now unless your a god mercy vs potatoes
it’s a toss up between Launch Vallyrie and Launch Brig.
Valk was so busted it was utter ridiculousness, turning fight into 10v6 (played well vanilla vall had FOUR insta rezzes).
Brig was less overpowered, but while mercy was so busted she made every playstyle viable, brig essentially pigeonholed the game into goats.
So, valk was objectively more overpowered, but brig did more lasting damage to the game.
At least valk was well designed in theory, so balancing it made it ok, but brig was fundamentally broken.
As for 35 ironclad bastion… yes, he was busted, but he lasted what? 3 days?
I know he lasted a whole month on console, but overall he was like a shot, painful but short.
Mercy and Brig were awful diseases that took a long time to heal

Doomfist. Its not even a contest. Launch brig still better than doomfist.
Vanilla McCree could 2 tap any 200hp hero at any distance and FTH did full damage which was really broken in the right hands, but release Brigitte was really crazy too so it’s hard to say which one was crazier. Mercy 2.0 was also really OP and a must pick since Ana was in a really bad spot at the time but she was still a pure healer so she didn’t carry games by herself unlike those 2 did.
Vanilla quickscope Widowmaker, Season 3 D.va and SA patch Hanzo are honorable mentions that probably fill the top 6
Initially Reworked Mercy hands down. Nothing died and if you count PTR she also became the DPS goddess due to her pistol buffs during Valk.

Doomfist has always been broken.
At launch he could rocket punch through a doorway and kill people on either side of the door.
He has gone through so many phases of being useless due to bugs and being OP because of the design and even now he can use any of his abilities and travel at full speed through slow effects which is broken with such low cooldowns.
The lead balance designer Geoff Goodman himself answered this question on the forums and said it’s Brig 1.0.
