Sometimes, a dead body will be within range but out of line of sight for resurrection, and the UI for resurrection (the name and face in the center of the screen) will appear, but pressing E will not actually allow you to rez that target until you gain line of sight on the target.
I’ve experienced this with a body on the other side of a lattice wall on Temple of Anubis (on the right side of the platform where most people hold for Point A) as well as a soft corner in Volskaya (see video below).
I’d hoped to capture this by getting it in a highlight, but it doesn’t seem to show in the highlight–just in-game. Maybe I’ll catch it in Shadowplay some day.
This video does NOT show the bug itself, but it does show the conditions under which I saw the bug in-game. When you see me pause at the corner in the video, I saw the rez UI on my screen and was trying to press E.
This does not seem to affect the actual mechanics of when you can or cannot rez someone in any way–it’s just a misleading UI cue.

I’ve experienced this countless times since the Halloween patch. It does affect my gameplay, since it will sometimes make it seem as though I can fly to someone who isn’t out of LOS. This is only an issue for me when they have died on a ledge or close to a ledge, as I would be able to fly to them otherwise. I essentially end up wasting time.
I reproduced this in the training range, recording with Shadowplay:
I’m following up because I’ve encountered this bug many more times in the course of the last few days, and sometimes it was a big deal. One situation like Prisinence mentions happened: A Symmetra had died on the bridge over the choke on the first stretch of Dorado, and I was trying to rez her from off the ledge, right before my team backed off. However, the UI suggested that I could start the rez when I actually didn’t quite have LoS on her–or maybe the fake rez start that happens with this bug prevented rez from starting when it actually could–so I didn’t get to rez her and the enemy team pushed and killed me while I was whirling about trying to get it to work.
