During a match I froze an Ana and lined up several great shots on her. Unfortunately, none of my shots seemed to go any farther than the tip of Mei’s gun and Ana remained unharmed until after my ult ended.

Hmm… So this has been happening at least since July. I haven’t really been playing Mei as much since they nerf’d her so that explains why I never saw it until now.
Yeah this is so infuriating. I have basically stopped using primary fire and ultimate all together. Much higher chance of getting a kill with secondary fire. Just plugging this thread where similar threads related to the problem are listed.
