How widowmaker helmet actually works?

Is is like VR pass-through or AR ?

I would say neither. Its basically a fancy sci-fi version of Military Nigh/Thermal-Helmets

It has needles that pierce into her skull , that’s what I imagine

The same way Sam Fisher’s helmet works.

thats what i image for basically her whole OW2 suit (to make some sense of all those senseless spikes and useless armorplates).
on the OW2 visor itself, I have written a short text snippet for my fanfic some time ago:

She inspects the equipment and criticizes the visor, the general weight, clunkiness and the restricted movement, as well as the fact that the suit is even more revealing. One of the scientists comes in. “This was not a question time for your criticism. This IS your new equipment. And it was not a request to put it on either.” He waves and several guards come in and hold Widow. The scientist takes the new visor and goes up to her. He takes off her old visor. “You are Talon’s property. You are not a human, you are a weapon and your only job is to obey our orders. This will ensure that it stays that way.” Then he turns the new visor over. Several sensors and needles flash on the inside. Widow’s eyes widen and she tries to tear herself away, but to no avail, the guards’ grip is like stone. He attaches the visor to her head, the sensors attach themselves to her head and the needle drills into her skull.
