Rewards screen says I finished in diamond and my career profile says I finished in diamond. My career profile also says that I won 21 comp games last season so I should have qualified for the “Competitive Player” prerequisite.

You needed to have won 25 competitive games in order to qualify for end-of-season competitive rewards. If you did this however, then you might have experienced a bug.
