Can't hear sound effects consistently

Since right before Patch 1.40, I have been unable to hear sound effects consistently - when I’m getting hit such as by symmetra turrets, ults on both the enemy team or my team (or I’ll hear the first second of the ult and it cuts out), certain abilities, etc.

I saw previous posts which were not resolved by reinstall. One solution worked temporarily by deleting the settings_v0 file but problems have started up again. Please help.

I tested this last night just to see if we could add anything else here. While not ideal, the audio sounded a bit better with Dolby Atmos enabled in-game, and enabling Windows Sonic for Headphones (Right click on Sound Icon. Spatial Sound > Windows Sonic for Headphones). I wasn’t able to get much better results with speakers, but hopefully this may help for headsets! The resolution will require a future Windows 10 update: September 10, 2019—KB4515384 (OS Build 18362.356) - Microsoft Support

Microsoft did offer some workarounds in the link above.
