I got a leaver status and won a game in QM to get out of it, however the game never recognized it and I’m still flagged with the leaver status.

Are you sure it was just 1 game? The penalty may require you to win up to 4.

If you disconnected during that penalty game (even right at the end), it might not count as a win despite your team winning the match.
Also, being disconnected for 10+ minutes and then reconnecting can also lead to a game being declared a Loss for you, even if it was a Win for other teammates.
Just some possibilities.
