In the Hellbound Surge event, you can upgrade the QUALITY of Hellbound Desire or Bottled Hope 5-star legendary gems.
Please note: Rank and quality are different things. A 5-star gem can be rank 1-10, quality 2-5 (this is the number of “active” stars out of the 5 it can have). The event can give up to 22 Rift Talismans. With 1, you can turn a Hellbound Desire 2/5 into a 3/5, with 7 more, you can turn the 3/5 gem into a 4/5. If you have a 4/5 gem, you’d need all 22 Talismans to turn it into a 5/5. But the RANK will be the same, if the “original” 2/5 gem was rank 4, it will still be rank 4.

Therefore, this event is only meant to awaken the stars within the gem, not to increase the gem’s rank itself. Since the event rules have been provided here, I suggest you review them to clear up any doubts you may still have.
