Yep. Just happened to me in Halls of the Damned where the dungeon was fully cleared and he was dead and I even screenshot him dead, but the quest objective wasn’t completed and I got no loot and didn’t receive the Codex of Power for the dungeon either (still in World Tier 2 unlocking all the Codex of Powers).
This happened less than 24 hours ago in a different dungeon, with the freaking Tomb Lord. He is broken and in way too many dungeons and other events as a “boss”.

Same thing happened to me twice today…
First in some Nightmare Dungeon which I don’t remember and now again in NM Dungeon Luban’s Rest.
Similiar case… Boss dies also when teleporting to town and back I see the death animation again.

Same happened to me earlier, I just killed him in “Carrion Fields” but he is not counting as dead, and I cannot teleport back to town, so this is literally a ‘make me wanna log out’ type of situation. NM Tier 30 Carrion Fields

dungeon was a whisper
was also in a blood pact zone (green zone)
Game did disconnect and reconnect half way thru the dungeon and all my season achievements spammed clan chat on reconnect
If I leave the dungeon and re-enter the tomb lord does his death animation
If I walk away from the boss area I see the skull icon on map untill i get close again then i get the death animation again,
Its like the tomb lord is playing possum rofl

Same thing just happened to me in Whispering Vault, Tomb Lord quest won’t complete even though boss is dead.

Same here in Mercy Reach, so i cannot get aspect for Codex where this guy is the final boss? Come on Blizzard you managed to destroy things that worked in season 1???

This literally just happened to me. This always happens to me against this boss! and the constant freezes that happen in the game are also irritating.

Same thing just now in Crusaders’ Cathedral, it is in a whisper zone but it is not the whisper dungeon that is active.

Still happening, awakened crusaderès chamber, toom lord can bug out and not give dungeon completion.
Happened once with the power Accursed Touch and once with poisons. (although in both instanced target was poisoned and cursed)

If I run out and back in, he makes the death animation but never triggers his defeat.

Happened in Ossuary of Echoing Anguish.
I’m on a Conjuration Sorc. So I had Hydra, Lightning Spear, and Ice Blades, all up and was smacking the boss with Arc Lash. It died and it didn’t complete.
Left the dungeon by porting to exit, came back in, it redid the death animation but still didn’t register. TP’d out and back in, same thing.

Same issue. Has happened to me at least 3 times now where you kill him but game does not give credit. I killed him very fast this last time don’t know if that has something to do with the problem or not. Carrion Fields NM
On top of that I’m not able to port out or use the leave dungeon on action wheel.

He must be playing dead or taking a nap. Tried to wake him and he just lays there.
