She can’t disappear if we want a Mephisto expansion, or else it would need a cheesy explanation like she got mugged.
No. He wasn’t in any D3 cinematic beyond the funeral. Last seen hanging out in the Act 5 town talking with Leoric about rebuilding the Horadrim order. He likely ages far slower than a regular mortal and is still alive. Leoric muses on it throughout the D4 campaign.

Tyrael is back in the heavens after the events of D3.
So, unless the heavens let her in with a PRIME EVIL in a soulstone, (no chance in hell, no pun intended) just to meet tyrael, ain’t gonna happen.
Also Tyrael is on the heaven’s council as a mortal. Heaven chose to let him stay there for the sacrifices he did during D3
Does it matter? At least she’s armed (mostly).
She is probably trying to figure out what’s west of westeros.

I don’t know if we’ve ever officially stated it or not, but it’s 100% a spherical celestial body with at least 1 moon, not a flat plane with edges.
