The realmwalker is there when the hatred rising icon appears. It’s usually at 30 minutes past the hour. The event spawns after 5 minutes. Go to the region when the icon appears and look for the gray skull with horns - it will show on your map and minimap. I would post screenshots but you can’t post them here. This is most definitely a bug.
