Same here, the Wheel (E) keeps losing the mouse up/down, so cant change screens there, the dismount breaks, and for some reason my 1 hotkey got reassigned to my M5 mouse button. Completely random.
It is happening to me also. Dismount, emote wheel (E) and force move are broken. it is very frustrating.
Confirming it’s a problem for me, too. My #2 action button is right-click on mouse and it is always unbound when I log in, requiring me to bind it again every time I play.
Can confirm I have the same issue. Every time I log in my Core skill button and evade are not bound. As a first time D4 HC player this got me almost killed multiple times…

I run into the same issue 
Also still having this issue all season. Every time I log out and back in, 2 of my keybinds become undone. Thus my first 2 skills are unbound from key assignments.
This has happened to me since the start of the season - specifically to the number 2 and 3 keys on my hotbar. It’s getting rather annoying.
