In addition to the bug with Skovos Sojourner, both the Tormented Slaughter (kill 1000 elites on Torment 12) and Echoing Elites (kill 1000 elites or bosses in Echoes) achievements appear bugged, but still obtainable.
I and a few other Xbox players have been blasting since release, and I was certain I was over 1000 T12 kills by a few days in, but no one was able to unlock that achievement. I assumed it was bugged and forgot about it, until this evening.
I was running pits and received a message in chat that Tormented Slaughter was close to completion, which should mean I only had 100 kills left. I quickly killed another 100, and it did not pop. I then had an idea that the numbers might be wrong. I killed another 400 elites and the achievement finally popped.
So, it appears Tormented Slaughter currently requires 5000 elite kills on T12 instead of 1000 as the tooltip says.
Iāve also been working on Echoing Elites all weekend - offering to carry people through their Traces of Echoes. I recorded them and watched them back and am certain Iām now over 1000 elites/bosses killed in Echoes as well, but no dice for that achievement either. I have a hypothesis that that achievement may also require 5000 instead of the 1000 suggested on the tooltip.
Edit: I also unlocked Echoing Elites today on Xbox. Iād estimate it took 2000-2500 kills or so. Quite a bit more than the achievement description suggests.

Yes ā I can fully confirm everything that was mentioned.
I have personally killed well over 1,000 elites on Torment 12, and the progress is clearly not being tracked correctly, as the trophy has still not unlocked. This is not an isolated case ā multiple players are reporting the exact same issue, which strongly indicates that the tracking system for this achievement is currently broken.
Additionally, all three of the mentioned trophies appear to be bugged. At this point, no console players on PlayStation or Xbox have been able to unlock them, despite meeting the requirements. This is a widespread problem affecting progression and completion for many players.
Blizzard needs to address and fix these issues as quickly as possible, as they are directly impacting the ability to complete the game and earn its achievements.

Same issue for me on PS5. The counter is not tracking all Elite kills.

Well Iāve gotten the āTormented Slaughterā on PS5, which is definitely NOT 1000 elite kills on T12 as the tooltip suggests. It just randomly popped one day.
Iāve done 12+ runs of Echoing Hatred (tier 140+ finish) for āEchoing Elitesā and feel this should have popped by now.
- Both of the above achievements have incorrect tooltips.
The āSkovos Sojournerā achievement is outright bugged and unobtainable, as itās been verified that the cow island does not contribute to this, leaving most people stuck at 65/67 locations found.

This issue is also affecting Steam achievement progress as well. I am tracking Tormented Slaughter and am well over 4,000 Elite kills in T12 (checked with seasonal tracker which was completed days ago - I started T12 with around 1k on the tracker and finished at 6,666 kills without this achievement unlocking and this was a few days ago). I believe this may only be tracking the āyellowā named elites and not blue/purple (even though these are technically āeliteā mobs as well since you can see this by testing with the aspect that gives barrier).
I did not unlock Echoing Hatred until Paragon 260 (another potential issue) but I do wonder if itās the same thing there. I havenāt played it enough to get up to 1k kills in either case just yet.
